for New Baby

Application Form

  • 個人での御祈祷をご希望の場合は、事前に桑港寺(info@sokoji.org)宛にご連絡ください。

  • お子さまのお名前は、御祈祷札に記載するお名前をご記入ください。（日本語でのご記入を推奨いたします。）

  • お申し込みは、御祈祷希望日の2日前までにお願いいたします。

  • 御祈祷料は、グループ祈祷100ドル以上、個人祈祷200ドル以上を目安に、当日封筒に入れてお納めください。

  • お車でお越しの方は、桑港寺横のHemlock Streetにある駐車場をご利用いただけます。

  • 当日の服装やお持ち物に、特別な指定はございません。

  • お子さまの体調がすぐれない場合は、無理をなさらず、日程変更についてお気軽にご相談ください。

  • 御祈祷の所要時間は、約30分です

  • If you would like to arrange a private blessing ceremony, please contact Sokoji in advance at info@sokoji.org.

  • Please enter your child's name as you would like it to appear on the prayer tablet (ofuda). Japanese characters are preferred, if possible.

  • Please submit your application at least two days before your preferred ceremony date.

  • The suggested offering for the ceremony is $100 or more for a group ceremony and $200 or more for a private ceremony. Please bring your offering in an envelope on the day of the ceremony.

  • Parking is available in the temple parking lot on Hemlock Street, next to Sokoji.

  • There is no specific dress code, and no special items are required.

  • If your child is not feeling well, please feel free to contact us to reschedule the ceremony.

  • The ceremony will take approximately 30 minutes.
